Description
We are looking for a Nurse Practitioner to join our growing team. The Nurse Practitioner must possess excellent communication and problem-solving skills as well as the ability to work independently. Responsibilities include providing primary care to patients, diagnosing and treating acute and chronic illnesses, ordering and interpreting laboratory tests and diagnostic imaging studies, performing injections and other treatments, providing patient education, and referring to specialists when appropriate. The ideal candidate must have a current, valid nursing license and CPR certification, as well as experience administering injections and providing patient education. Additionally, the Nurse Practitioner should possess strong clinical judgment and a commitment to providing high quality, patient-centered care.
Responsibilities
• As a Nurse Practitioner at Rezi, responsible for providing high-quality healthcare services to patients in a variety of settings
• Perform thorough physical exams and accurately diagnose patients’ conditions
• Prescribe medications, order and interpret diagnostic tests, and provide patient education and counseling
• Develop and implement treatment plans, evaluate and monitor patients’ progress, and modify treatment plans as needed
• Communicate effectively with the patient’s family, physicians, and other healthcare professionals to ensure the patient receives the best care possible
• Promote preventive health care, patient education, and self-care
• Maintain patient records, document patient progress, and charge for services as necessary
• Manage difficult patient cases and triage, stabilize, and refer patients to specialists as appropriate
Requirements
• Must hold an active and unrestricted license to practice as a Nurse Practitioner in the state of practice
• Must have 1-2 years of experience as a nurse practitioner
• Must have a valid DEA certificate
• Must have a current CPR/BLS certification
• Must have strong clinical assessment and decision-making skills
• Must have strong interpersonal and communication skills
• Must have the ability to work independently and collaboratively as part of a team
• Must demonstrate the ability to think critically and creatively to solve complex problems
• Must be knowledgeable of current medical standards and practices
• Must be comfortable using electronic medical records (EMR)

Description
We are looking for an experienced and compassionate Nurse Practitioner to join our team at Rezi. The Nurse Practitioner will provide patient-centered healthcare services including diagnosis and treatment of acute and chronic medical conditions. The ideal candidate will have a minimum of two years of experience in a clinical setting.The Nurse Practitioner will be responsible for providing high quality healthcare services, including injections, CPR proficiency, patient education, preventative care, and other services as needed. They will also be responsible for ordering and interpreting appropriate diagnostic tests, developing a plan of care, and providing education and counseling to patients and their families. The Nurse Practitioner must be able to work collaboratively with physicians and other healthcare team members to ensure the best care possible for our patients.
Responsibilities

• Assess patient health by conducting physical exams, taking medical histories, and ordering laboratory tests and diagnostic procedures
• Diagnose and treat acute illnesses, infections and chronic conditions
• Analyze test results, identify deviations from normal and make appropriate treatment decisions
• Develop individualized healthcare plans and make recommendations for changes in lifestyle to improve patient’s health
• Educate patients and their families about health maintenance, disease prevention and treatment options
• Prescribe medications and other treatments and provide follow-up care
• Communicate with other healthcare professionals to coordinate optimal patient care and refer to other healthcare specialists as needed
Requirements
• Graduate of an accredited Nurse Practitioner program
• Current state licensure as a Nurse Practitioner
• Minimum of 3 years of experience as a Nurse Practitioner
• Advanced knowledge and experience in providing direct patient care
• Ability to assess, diagnose, and manage acute and chronic health issues
• Ability to provide patient education and health promotion
• Ability to collaborate with other healthcare professionals
• Knowledge of relevant laws, regulations, and guidelines related to healthcare
• Excellent communication, interpersonal, and organizational skills
• Ability to exercise sound judgement and critical thinking
